Funeral directors like you play a critical role in protecting the public’s health by ensuring that death certificates are filled out completely and accurately. Data from these death certificates are crucial to help experts identify and respond to disease trends. However, previous research has shown that American Indian and Alaska Native (AI/AN) people are often “misclassified” on death certificates.

The National Council or Urban Indian Health (NCUIH) would like to learn from your expertise on working with families and filling out death certificates. With the support of the Centers for Disease Control and Prevention (CDC), NCUIH is seeking input from funeral directors across the nation to better understand how the collection of demographic information works in practice.
