Requirements:

Transport of Human Remains

1. The same documentation that applies to the United Kingdom, applies here however the following is requested to be faxed to the receiving funeral home and then travel with the remains
2. Certified copy of Death certificate
3. Burial/Transit Permit
4. Passport of decedent
5. Embalmer's affidavit      
6. Letter of preparation on funeral home letterhead
7. Letter of non-contagious disease from Health Department
8. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
9. No requirements on casket
