Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit with raised seal
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from Department of Health
6. Confirmation from Civil Registrar or Religious Community in charge of the cemetery in Switzerland that there is burial space available for the remains. This confirmation may be telefaxed, faxed, etc
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
8. No casket requirement
9. Zinc lined shipping box
10. No consulate fee or inspection
