Requirements:
Transport of Human Remains
- Certified copies of Death certificate (3)
- Burial/Transit Permit
- Passport and green card of decedent
- Embalmer's affidavit
- Letter of non-contagious disease from State or County Health Official
- Letter of non-contagious disease from attending physician with cause of death
- Letter from family on funeral home letterhead requesting funeral home and consulate make necessary arrangements to have remains sent back to Spain
- Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
- The family MUST go to the Consulate to ‘Register’ the death (if Spanish citizens) Consulate will allow the family to present the papers on behalf of the funeral home
- All paperwork should be translated to Spanish before sending to the Consulate
- Metal Sealer Casket or zinc lined wood casket
- Wood shipping case
- No Consul inspection required

- Certified copy of the death certificate
- Cremation Certificate
- Original Passport of decedent
