Requirements:

Transport of Human Remains

1. Certified copies of Death certificate (3)
2. Burial/Transit Permit
3. Passport and green card of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from State or County Health Official
6. Letter of non-contagious disease from attending physician with cause of death
7. Letter from family on funeral home letterhead requesting funeral home and consulate make necessary arrangements to have remains sent back to Spain
8. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
9. The family MUST go to the Consulate to ‘Register’ the death (if Spanish citizens)       Consulate will allow the family to present the papers on behalf of the funeral home
10. All paperwork should be translated to Spanish before sending to the Consulate
11. Metal Sealer Casket or zinc lined wood casket
12. Wood shipping case
13. No Consul inspection required

Transport of Cremated Remains

1. Certified copy of the death certificate
2. Cremation Certificate
3. Original Passport of decedent
