Transport of Human Remains

1. Certified copies of Death certificate (2)
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from state or county official
6. Letter of preparation on funeral home letterhead including the procedure and contents of casket. The letter also needs to state the outer wood shipping container and have the consignee’s information including telephone number
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) list final burial place in Slovakia
8. If death was the result of a murder or homicide, consulate wants a copy of the police report
9. Sealer casket
10. Wood overseas container
11. Consul fees $86.00/ no inspection required

Transport of Cremated Remains

1. Certified copy of the death certificate
2. Cremation Certificate
3. Written authorization from cemetery in Slovakia that allows cremains to be buried
4. Metal sealed Urn
5. $24.00 money order
