Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from local health authority
6. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
7. Hermetically sealed Casket
8. New airtray is ok
9. No consular fee or inspection
10. A copy of all documents to be kept at the Consulate

Transport of Cremated Remains

1. Certified copy of the death certificate
2. Cremation Certificate
3. A statement from the funeral home that the urn has been hermetically sealed and contains only the remains of the deceased
4. One copy of each document to be kept at the Consulate
5. No Consular fee or inspection
