1. Certified copies of Death certificate (2)
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavits (2) notarized
5. A letter from the doctor stating the cause of death, notarized
6. Letter of non-contagious disease from local health authority with seal and notarized
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) name and place of arrival
8. Zinc lined shipping box
9. The Embassy prefers the family make the request for shipment before the documents are presented
10. A letter from the competent USA authorities (police, customs) issued after the identification of the body and control and seal of the casket
11. Fee (money order) is $55.00
