Transport of Human Remains

1. Certified copies of Death certificate (3)
2. Burial/Transit Permits (3)
3. Passport of decedent
4. Embalmer's affidavits (3)
5. Letters of non-contagious disease from State or County Health Office (3)
6. Letters of non-contagious disease from attending physician (3)
7. Copies of “declaration of death” including the declaration of the possessing the final place of disposition (3)
8. Letter of preparation on funeral home letterhead
9. If death was a suicide or accident, copies of the police report are required (3)
10. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
11. Consulate prefers that the documents are faxed ahead of time so they may be translated
