Requirements:

Transport of Human Remains

1. Puerto Rico is a possession of the United States. All requirements for shipping within the U.S. apply but the following documents are requested as well
2. Certified copy of Death certificate
3. Burial/Transit Permit
4. Letter of preparation on funeral home letterhead
5. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
6. No casket requirement, combo unit is OK
