Requirements:
Transport of Human Remains
- Before documentation through the Consulate can commence the family will need to obtain permission from Poland to ship the remains. The following is required for permission: 1) Full name of the deceased; 2) Dates and places of birth and death; 3) Full names of the parents; 4) Exact cause of death; 5) Place of burial in Poland; 6) Name and address of person in Poland accepting responsibility for the family in the USA. AFTER this is obtained the remaining documents through the Consulate are requites
- Certified copies of Death certificate (2)
- Approval of the funeral home in Poland
- Burial/Transit Permit
- Passport of decedent
- Embalmer's affidavit
- Letter of non-contagious disease from State or County Health Office
- Letter from the registrar that they have no objection to having the remains shipped to Poland
- Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
- All documents must be notarized

Transport of Cremated Remains
- Certified copies of Death certificate (2)
- Certificate of Cremation
- A certificate issued by a funeral home, stating that the cremated remains are of the person named in the death certificate and are enclosed in an urn
- The same permit from a funeral home in Poland, as listed above, is also required to ship the cremated remains
