Requirements:

Transport of Human Remains

1. Certified copies of Death certificate (2)
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from County Health Office, notarized and authenticated
6. Letter of preparation on funeral home letterhead, notarized and authenticated
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
8. Sealer Casket
9. Air tray
10. No consul inspection, legalization fee is $32 per each document

Transport of Cremated Remains

1. Certified copy of Death certificate
2. Cremation Certificate (notarized and authenticated by County Clerk)
3. Fee $32.00 per document
