Requirements:
Transport of Human Remains
- Certified copy of Death certificate
- Burial/Transit Permit
- Passport of decedent
- Embalmer's affidavit
- Letter of non-contagious disease from local State or County Health Official stating that the body did not die from a communicable disease and that there were no contagious diseases present in the area at the time of death, signed and notarized
- Letter of preparation on funeral home letterhead, letter must state remains were placed in a metal sealer casket, signed and notarized
- Certificate of Authentication for Notary from County Clerk
- Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
- Metal Sealer Casket
- A new wood bottom base air tray
- Consulate documentation fee is $100.00
