Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit (notarized)
5. Letter of non-contagious disease from local health authority
6. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) Name of deceased with age and place of birth; 5) Date, place and cause of death 6) Nationality of the deceased 7) Name of cemetery in Mauritius and proposed time of burial
7. Hermetically sealed casket
8. Permission for the shipment must be obtained from the permanent Secretary, Ministry of Health, Emmanuel Anquetil Building, Sir Seewoosagur Ragoolam St., Port Louis, Mauritius
