Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Macedonian passport of decedent
4. Embalmer's affidavit on funeral home letterhead
5. Letter of non-contagious disease from Municipal Health Department stating there was no communicable disease in the community
6. Letter of non-contagious disease from the attending physician that the deceased did not die from a communicable disease
7. A letter from the funeral home requesting permission to ship remains to Luxembourg
8. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary and final point of destination in Macedonia (usually Skopie); 3) consignee's name, address, and telephone number;
9. No requirements on casket
10. Zinc lined wood shipping box
11. Fees $21.00
12. If death is of violent or accidental nature; consulate wants a certified copy of police report
