Lithuania - Shipping Requirements

Requirements:

Transport of Human Remains

  1. Certified copy of Death certificate
  2. Burial/Transit Permit
  3. Passport of decedent
  4. Embalmer's affidavit
  5. Letter of non-contagious disease from County or State Health Department
  6. Permit to bury remains in Lithuania (obtained from local authorities in Lithuania)
  7. Notarized statement that only human remains are in the container, description of the casket with materials, color and dimensions and if remains were contagious, that they have been properly disinfected
  8. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
  9. Hermetically sealed casket
  10. All wood shipping container with zinc liner
  11. No consul inspection of remains is required
  12. No consul fee (all expenses on mailing and travel must be compensated)

Transport of Cremated Remains

 

  1. All the same documentation must be adjusted for cremation
  2. Real urn with a metal lined wood box
