Requirements:
Transport of Human Remains
- Certified copy of Death certificate
- Burial/Transit Permit
- Passport of decedent
- Embalmer's affidavit
- Letter of non-contagious disease from County or State Health Department
- Permit to bury remains in Lithuania (obtained from local authorities in Lithuania)
- Notarized statement that only human remains are in the container, description of the casket with materials, color and dimensions and if remains were contagious, that they have been properly disinfected
- Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
- Hermetically sealed casket
- All wood shipping container with zinc liner
- No consul inspection of remains is required
- No consul fee (all expenses on mailing and travel must be compensated)
Transport of Cremated Remains
- All the same documentation must be adjusted for cremation
- Real urn with a metal lined wood box
