Transport of Human Remains

1. Certified copies of Death certificate (2)
2. Burial/Transit Permits (2)
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease (2)
6. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
7. 2 copies for each document: 1 set for any escorts; 1 set for airline
8. Hermetically Sealed casket
9. Air tray as outer shipping container

Transport of Cremated Remains

1. Certified copies of Death Certificate (2)
2. Certificates of Cremation (2)
3. Non contraband affidavits (2)
4. Urn packed in wood box
