Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from local health authority w/ seal
6. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
7. Metal Sealer
8. Wood bottom air tray
9. Inspection by consulate representative
10. For U.S. citizens desiring to be buried in Kyrgyz Republic: 2 letters are required: 1 from relatives in town where burial is requested that will accept financial responsibility and 1 from local authorities stating there is no objection to the burial in the specific cemetery to be obtained by family members in the town of burial

Transport of Cremated Remains

1. Certified copy of Death Certificate
2. Cremation Certificate
3. Urn packed in wood box
4. Letter from family members in Kyrgyz Republic who will accept shipment
5. Inspection of urn and sealing
