Transport of Human Remains

1. Certified copy of Death certificate stating cause of death (1)
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit (notarized)
5. Letter of preparation on funeral home letterhead
6. Letter of non-contagious disease from health department
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
8. No requirements on type of casket
9. Zinc lined shipping box
10. No Consul inspection or fee

Transport of Cremated Remains

1. A certified copy of the death certificate stating cause of death
2. Cremation Certificate
