Requirements:

Transport of Human Remains

1. Certified copies of Death certificate (2 – must show cause of death)
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit      
5. Letter of non-contagious disease from health department with raised seal
6. Original police report when death is not of natural causes
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
8. Sealer casket or Ziegler case
9. Wooden outer shipping box with handles (no inner liner)
10. Procedure: Send the documents to the Consulate for being verified and registered. They will be processed the same day and sent back with FedEx (do not forget to enclose a check or money order for $20.00 payable to “The Interests Section of the Islamic Republic of Iran” for the return of documents by FedEx). Do not fax documents – all documents must be original. The Interests Section of the Islamic Republic of Iran is not responsible for any charges incurred as a result of the shipment of body unless a written permission is issued by the Consulate.
