Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit      
5. Letter of non-contagious disease from Dept. of Health (raised seal)
6. Funeral home letter stating: 1) only remains are inside the casket and that the casket has been hermetically sealed within a zinc lined shipping container (notarized); 2) flight itinerary; 3) consignee's name, address, and telephone number;      
7. If homicide, a letter of permission to ship from the Police Department involved in the investigation and the District Attorney’s Office
8. No casket requirements
9. Zinc lined outer wood shipping box
10. All documents require one photocopy
11. No Consul inspection required. Consulate fee is $60.00 in cash

Transport of Cremated Remains

1. Certified copy of Death certificate
2. Certificate of Cremation
3. Passport
