Requirements:
Transport of Human Remains
- Certified copies of Death certificate (2)
- Burial/Transit Permit
- Passport of decedent
- Embalmer's affidavit
- All documents should be copied and also faxed to the receiving funeral home in Grenada
- Letter of non-contagious disease
- Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number;
- No restrictions on type of Casket
- The outside of the casket shall bear an immovable plaque or other appropriate marking, in a conspicuous place, indicating the name, age and place of the final destination of the body
- Where an inquiry is pending and the death certificate cannot be obtained, a copy of the coroner’s report which must clearly state the cause of death is acceptable
