Requirements:
Transport of Human Remains
- Certified copy of Death certificate with raised seal
- Burial/Transit Permit
- Passport of decedent (expired is ok)
- Embalmer's affidavit (notarized)
- Letter of non-contagious disease (notarized)
- Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) cemetery name and location 5) statement that the casket only contains the deceased and has no monetary value
- Casket – hermetically sealed
- All wood shipping container
- The person or the funeral home is wholly responsible for advising the customs authorities of the manner, date and time of arrival of the shipment, and the receiving funeral home must be present at customs to facilitate clearance of the remains. There is no legalization procedure at the Consulate. Simply fax documentation to receiving funeral home so they can forewarn inbound customs and send original documentation with the remains
