Requirements:

Transport of Human Remains

1. Certified copy of Death certificate (2)
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit (notarized)
5. Letter of non-contagious disease
6. Casket - sealer-type metal
7. Zinc-lined wood shipping box
8. Authorization by the Mayor of the city where the deceased is to be buried must be obtained

Transport of Cremated Remains

1. Certified copies of Death Certificate (2)
2. The urn must have a metal plaque with the name of the deceased, consil will inspect and seal the urn (urn to be shipped in a rigid outer container)
3. Cremation Certificate
4. Consulate supplied form signed by the shipping funeral director requesting the shipment
