2. Photocopy of the ID of the deceased
3. Burial/Transit Permit
4. Embalmer's Affidavit
5. Letter of non-contagious disease
6. Funeral home letter stating: 1) That only the embalmed remains are in the casket, 2) That the casket has been placed in metal casket and then into a zinc-lined wood shipping box and 3) That the box has been hermetically sealed and is odor free
7. Casket - Metal, sealer-type
8. Zinc-lined wood shipping box
9. $45.00 Consulate Fee
