Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Medical certificate stating the cause of death
3. Burial/Transit Permit
4. Passport of decedent
5. Embalmer's affidavit w/ statement claiming the casket contains ONLY human remains on letterhead and that the body had been coffined in accordance with local instructions
6. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
7. Hermetically sealed casket
8. Inside of the casket should be metal, the outside coffin should be a regular coffin for transportation

Transport of Cremated Remains

1. Certified copy of Death certificate
2. Urn must be sealed
