Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit      
5. Letter of non-contagious disease signed, notarized and authenticated
6. Photocopy of complete funeral bill
7. Photocopy of funeral home registration from Dept. of Health and copy of embalmer’s license
8. Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number;
9. Sealer Casket
10. Wood-bottom airtray
11. Consul inspection is required. Fees are $160.00 plus 3% total funeral expense
