Requirements:
Transport of Human Remains
- Certified copies of Death certificate (3)
- Burial/Transit Permit
- Passport of decedent (note: if not a Greek passport you must have an apostille on the Death Certificates) Apostille and Death Certificate have to be sent out to be translated by a translation service designated by the consulate.
- Embalmer's affidavit (signed and notarized)
- Letter of non-contagious disease (signed and notarized)
- Funeral home letter (signed and notarized) stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) name of cemetery and the town in which it is located
- No restrictions on type of Casket
- Screw down steel liner & Outer wood box required
- A copy of the funeral bill
