Requirements:
Transport of Human Remains
- Certified copy of Death certificate
- Burial/Transit Permit (must bear raised seal of the health department)
- Passport of decedent
- Embalmer's affidavit (must be notarized and authenticated by the county clerk)
- License Verification Letter (funeral home letter stating the embalmer's license number with a photocopy of his/her license along with a photocopy of the funeral home's registration - this letter must be notarized and authenticated by the county clerk)
- Letter of non-contagious disease
- Funeral home letter stating: 1) remains were properly embalmed and prepared for international shipment; 2) that only the remains are inside the casket; 3) flight itinerary; 4) consignee's name, address, and telephone number; 5) cemetery name and location (this letter must be notarized and authenticated by the county clerk)
- All documents need Apostille from the State Department
- Casket - sealer-type metal
- Air tray (wood base)
