Requirements:

Transport of Human Remains

1. Certified copy of Death certificate (2)
2. Burial/Transit Permit (must bear raised seal of health department)
3. Passport of decedent
4. Embalmer's affidavit (notarized)
5. Letter of non-contagious disease (must bear raised seal of heath department)
6. Funeral home letter stating:

• • Only the remains are inside the casket;
  • Flight itinerary;
  • Consignee's name, address, and telephone number;
  • Cemetery name and location (notarized)

7. Casket - sealer-type metal
8. Air tray (wood base)
9. Family must go to Consulate to register the death. That family members needs to bring the passport with them at that time.       However, the registration process can be done by the family in Chile if they’re returning for the burial
10. No consulate inspection; Consulate fees: $12.00 per document
