3. Passport of decedent
4. Embalmer's affidavits (5)
5. Letters of non-contagious disease from local health authority (5)
6. Funeral home letters (5) stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
7. Hermetically sealed casket
8. Air tray

Transport of Cremated Remains

1. Certified copies of Death certificate (7)
2. Cremation Certificates (5)
3. Urn to be placed in unbreakable outer container for shipping
