Requirements:
Transport of Human Remains
- Relatives must obtain a letter from the cemetery in the Ukraine stating that the family owns a grave, crypt, etc. The consul MUST have this first
- Certified copy of Death certificate authenticated by County Clerk and affixed with Certification from State Department
- Burial/Transit Permit signed, notarized and authenticated by County Clerk and affixed with Certification from State Department
- Passport of decedent
- Embalmer's affidavit
- Letter of non-contagious disease from County or State Health Department signed, notarized and authenticated by County Clerk and affixed with Certification from State Department
- Letter of non-contagious disease from attending physician signed, notarized and authenticated by County Clerk and affixed with Certification from State Department
- Burial/Transit Permit
- Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
- Sealer Casket
- Zinc lined wood outer shipping box
- Body must go to the consulate for wax seal
- Money Orders ONLY: One for $240.00 One for $100.00
