Ukraine - Shipping Requirements

Requirements:

Transport of Human Remains

  1. Relatives must obtain a letter from the cemetery in the Ukraine stating that the family owns a grave, crypt, etc. The consul MUST have this first
  2. Certified copy of Death certificate authenticated by County Clerk and affixed with Certification from State Department
  3. Burial/Transit Permit signed, notarized and authenticated by County Clerk and affixed with Certification from State Department
  4. Passport of decedent
  5. Embalmer's affidavit      
  6. Letter of non-contagious disease from County or State Health Department signed, notarized and authenticated by County Clerk and affixed with Certification from State Department
  7. Letter of non-contagious disease from attending physician signed, notarized and authenticated by County Clerk and affixed with Certification from State Department
  8. Burial/Transit Permit
  9. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
  10. Sealer Casket
  11. Zinc lined wood outer shipping box
  12. Body must go to the consulate for wax seal
  13. Money Orders ONLY: One for $240.00 One for $100.00
