Requirements:

Transport of Human Remains

1. Certified copies of Death certificate (2)
2. Burial/Transit Permit with raised seal
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from State or County Health Official
6. Letter of non-contagious disease from attending physician stating cause of death
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
8. A duplicate must be made for every document including notary
9. No casket requirement
10. Screw down lead liner with outside wood shipping case
11. All documentation must be sent to the Syrian Embassy in DC by Fed Ex WITH a PREPAID return envelope
12. No consulate fee or inspection
