South Africa - Shipping Requirements

Requirements:

Transport of Human Remains

  1. Certified copy of Death certificate
  2. Burial/Transit Permit
  3. Passport of decedent
  4. Embalmer's affidavit
  5. Letter of non-contagious disease from Health Department
  6. Letter of non-contagious disease from physician (must state cause of death)
  7. Letter from Department of Health stating that a post mortuam examination of the body does not reveal the presence of any communicable disease
  8. Letter from the funeral home requesting permission to ship remains to South Africa with complete flight itinerary
  9. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
  10. Sealer Casket
  11. All wood outer shipping container
  12. Consul will issue a letter of permission to ship. No inspection or fee
