Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent or deceased certificate on Serbian citizenship and a document serving as proof of identity (ID card, driver’s license, old passport, etc)
4. Embalmer's affidavit
5. Letter of non-contagious disease from local health authority
6. A certificate issued by the funeral home stating that the human remains were not previously buried and were placed in a hermitically sealed metal/zinc box
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) name and place of arrival
8. Hermetically sealed casket
9. Money order in the amount of $3 payable to the Embassy of the Republic of Serbia as a fee for issuing the document
10. Return self-addressed envelope with paid postage for registered mail
