Transport of Human Remains

1. Certified copy of Death certificate with letter of exemplification Notarized, authenticated by County Clerk’s office and fixed with an Apostille from the U.S. State Department
2. Burial/Transit Permit
3. Photocopy of passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from state or county health official, signed, notarized and authenticated by the county clerk’s office with an Apostille from the U.S. State Department
6. Letter of preparation on funeral home letterhead containing procedure and contents of casket signed, notarized and authenticated by the county clerk’s office with an Apostille from the U.S. State Department
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
