Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Application listing the name of deceased, date/place/cause of death, and that the body is embalmed, presented to the department of health
6. Letter of non-contagious disease from County Health Office, notarized and authenticated
7. Letter of preparation on funeral home letterhead, notarized and authenticated
8. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
9. Hermetically sealed Casket
10. All wood outer shipping container
