Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from County or State office, signed, notarized and authenticated by county clerk and affixed with apostille from State Department
6. Letter of non-contagious disease from attending physician, signed, notarized and authenticated by county clerk and affixed with apostille from State Department
7. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number;
8. Bill of lading number (Airway Bill number), details of destination and flight details should be faxed to the New Zealand funeral director who will advise New Zealand Customs and the appropriate airline
9. Hermetically sealed casket
10. Inside of the casket should be metal, the outside coffin should be a regular coffin for transportation
