Requirements:
Transport of Human Remains
- Certified copy of Death certificate
- Burial/Transit Permit
- Passport of decedent
- A letter from doctor stating the cause of death
- Embalmer's affidavit (notarized)
- Letter of non-contagious disease from Health Department (w/seal)
- Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) Name of deceased with age and place of birth; 5) Date, place and cause of death 6) Nationality of the deceased 7) Name of cemetery in Malta
- No requirement of casket
- Zinc lined shipping box, with 2” of sawdust on the bottom
- A statement from the funeral home stating that the remains have been placed in the zinc-lined shipping box
- Consul fee is $3.00 per document, plus an $18.00 fee for Telexes to and from Maltese Health Authorities
