Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent     
4. Letter of non-contagious disease from attending physician
5. Funeral home letter stating: 1) only remains are inside the casket 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) cemetery information. Must have City, State and Exact plot location with authorization for Arab
6. Deceased’s Hebrew AND Father’s name
7. Steel-lined wood box
8. Relatives in Israel to contact (name, address and phone)
