Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from State or County health office
6. A letter from the family requesting permission
7. Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number;
8. Sealer Casket (w/ 3” of peat, sawdust, or powdered charcoal moistened with an antiseptic solution
9. All wood shipping container with Zinc liner (secured by screws not more than 8” apart with 3 metal bands
