France - Shipping Requirements


Transport of Human Remains

1. Certified copies of Death certificate (2)

2. Burial/Transit Permit

3. Passport of decedent

4. Embalmer's affidavit

5. Authorization from Mayor of Town in France (could be faxed directly to the Consulate)

6. Letter of non-contagious disease from State or County health office

7. A letter from the family requesting permission

8. Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number;

9. Sealer Casket

10. All wood shipping container with banding

Transport of Cremated Remains

1. Certified Death Certificate of the deceased

2. Letter of request for shipment of remains. This letter can be prepared by either a family member or the Funeral Home authorizing the Consulate to seal the urn.

3. Certificate of Disposal of Human Remains delivered by the Health Department

4. Cremated remains should be in an urn which should be engraved with the Death Certificate references.

5. Consulate fee of approximately $30 to $60 to be paid in cash or by company check.

6. A family member or Funeral Home representative needs to come to the Consulate with the above mentioned unsealed urn and documents so that the Consulate can officially seal the urn. Please call ahead to make an appointment
