Ethiopia - Shipping Requirements

Requirements:

Transport of Human Remains

  1. Certified copy of Death certificate
  2. Burial/Transit Permit
  3. Passport of decedent
  4. Embalmer's affidavit
  5. Letter of non-contagious disease from State or County health office
  6. A letter from the family requesting permission
  7. Inspection by consulate representative
  8. Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; Letter must include the name of the deceased, place of birth & death, final disposition and next of kin authorizing arrangements
  9. No casket requirements – Ziegler is ok
  10. All wood shipping container with zinc liner
  11. No consul inspection is required and no fee. The Consul will issue permission letter
