Requirements:
Transport of Human Remains
- Certified copies of Death certificate (2)
- Burial/Transit Permit (notarized by County Clerk office)
- Passport of decedent
- Embalmer's affidavit (notarized)
- Letter of non-contagious disease from the attending physician
- Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number;
- A steel casket with a wood outer shipping container is preferred but a wood casket is acceptable if a steel screw-down liner is used for sealing purposes and the wood outer box
