Transport of Human Remains
- Certified copies of Death certificate (2 – needs to be translated)
- Burial/Transit Permit (w/ raised seal)
- Passport of decedent
- Embalmer's affidavit (notarized)
- Letter of non-contagious disease (notarized)
- Funeral home letter stating (in English and Spanish and notarized): 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) cemetery name and location
- Metal Casket – hermetically sealed
- All wood shipping container
- Documents to be translated to Spanish
