Dominican Republic - Shipping Requirements

Requirements:

Transport of Human Remains

  1. Certified copy of Death certificate
  2. Burial/Transit Permit
  3. Passport of decedent
  4. Embalmer's affidavit (notarized)
  5. Letter of non-contagious disease (notarized)
  6. Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) cemetery name and location
  7. Casket – hermetically sealed
  8. All wood shipping container is ok if sealer is used
  9. Documents to be translated to Spanish
  10. Consulate Fees: $150.00. Cash is needed as they do not accept checks. The Consulate will help translate to Spanish.
