Requirements:
Transport of Human Remains
- Certified copy of Death certificate
- Burial/Transit Permit
- Passport of decedent
- Embalmer's affidavit (notarized)
- Letter of non-contagious disease (notarized)
- Funeral home letter stating: 1) only remains are inside the casket; 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) cemetery name and location
- Casket – hermetically sealed
- All wood shipping container is ok if sealer is used
- Documents to be translated to Spanish
- Consulate Fees: $150.00. Cash is needed as they do not accept checks. The Consulate will help translate to Spanish.
