Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit (notarized and authenticated by the county clerk)
5. Letter of non-contagious disease FROM PHYSICIAN that must state the exact cause of death (notarized and authenticated by the county clerk)
6. Family MUST GO to the consulate and register the death with Brazilian authorities. Family must have the passport of the decedent and a death certificate.
7. Casket - sealer-type metal
8. Full flight itinerary
9. Zinc lined wood shipping box
10. Air tray (wood base)
