4. Embalmer's affidavit (2, notarized)
5. Letter of non-contagious disease (notarized)
6. Funeral home letter stating: 1) that only remains are inside the casket and that the body has been prepared according to international mortuary shipping rules; 2) flight itinerary; 3) consignee's name, address, and telephone number; 4) name of funeral home in Bosnia and Herzegovina along with their contact information; 5) cemetery name and location
7. Letter from Bosnian funeral home stating that they will accept the remains along with the name and location of the cemetery
8. Casket - sealer-type metal
9. Zinc-lined wood shipping box
10. Consulate fee of $21.00 (Est.)
