Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit (notarized)
5. Letter of non-contagious disease
6. Letter from physician stating: 1) cause of death; 2) cause of death was not contagious
7. Funeral home letter stating: 1) only remains are inside casket; 2) flight itinerary; 3) consignee's name, address, and telephone; 4) name of family member in Belize and their contact information; 5) cemetery name and location
8. All documents must be approved by the Chief Medical Office in Belize
9. Casket - sealer-type metal
10. Air tray (wood base)
