Requirements:

Transport of Human Remains

1. Certified copy of Death certificate
2. Burial/Transit Permit
3. Passport of decedent
4. Embalmer's affidavit
5. Letter of non-contagious disease from Health Department with raised seal
6. Casket - sealer-type metal
7. Wood shipping box
8. Consulate inspection required (do not place the remains inside in the casket prior to the consulate inspection)
9. No consulate fees

Transport of Cremated Remains

1. Certified copy of Death Certificate
2. Passport or other ID
3. Certificate of Cremation with the seal of the crematory
4. Notarized letter from the crematory stating the urn contains only cremains
5. The urn will be sealed with the Consular stamp
