Transport of Human Remains

1. Certified copies of Death Certificate (2)
2. Notarized and Authenticated Letter of Non-contagious disease
3. Letter of preparation and contents of casket on funeral home letterhead
4. Embalmers affidavit w/ flight information and consignees information on funeral home letterhead
5. Passport/Visa of decedent
6. Casket - Metal, sealer-type
7. Outside wood shipping container
8. Fax from consignee to the Austrian Consulate confirming pick up in Austria

Transport of Cremated Remains

1. Notarized Certificate of cremation stating container contains ashes of the deceased
2. Sealed metal urn with name of deceased and date of death engraved
